The need for information support in planning, monitoring and evaluation of health care interventions is a recognized fact by many health activists and international agencies as one of the factors which can lead to the achievement of the Millennium Development Goals (MDGs). Unfortunately, research reports covering the area of health information in developing countries is not uplifting.Much of the reports express the dismal state of the health information systems and its inability to respond to the information needs of the respective health system. The state of the health information system in Tanzania is not far from what is described in much of the research reports. As part of the health sector reforms, the Ministry of Health and Social Welfare (MoHSW) has embarked on a number of interventions in an attempt to revamp the health information system in Tanzania. Though the invested efforts led to some optimal achievements in terms of structure and availability of information for decision making, the health information system is still characterized as centralized, fragmented and inflexible due to its inability to respond to the ever changing needs of the health system.
